Application and End-of-Year Report

The state of Wisconsin has posted a draft of the ESEA Flexibility Waiver. If approved by USDE, this waiver would eliminate the current Supplemental Educational Services requirement starting Fall 2012. In lieu of the SES requirement, districts would be required to design extended learning opportunities in consultation with parents of eligible students.

Completed applications (all electronic submissions and a hard copy of Financial Soundness documentation) must be received by the Wisconsin Department of Public Instruction no later than 3:00pm on April 2. Late applications will not be reviewed. No FAXs will be accepted.

Completed applications include: 1) a submitted online application 2) a submitted Assurances and Financial Soundness survey 3) a submitted Certification of Debarment survey 4) evidence of Financial Soundness (original document in hard copy).

Completed end-of-year reports must be submitted electronically to the Wisconsin Department of Public Instruction no later than 3:00pm on August 1 of the reporting year. WI DPI will move forward with removal proceedings for approved SES providers who do not submit their end-of-year report.
