Completed applications (including both print and electronic) must be received by the Wisconsin Department of Public Instruction no later than 3:00pm on April 1. Late applications will not be reviewed. No FAXs will be accepted.

Completed end-of-year reports must be submitted electronically to the Wisconsin Department of Public Instruction no later than 3:00pm on August 1 of the reporting year. WI DPI will move forward with removal proceedings for approved SES providers who do not submit their end-of-year report.
