The SES provider application process in Wisconsin involves submitting information both electronically
and in print. Applications must include ALL electronic surveys (Assurances and Financial Soundness form,
Certification of Debarment form, and audit requirements) AND online information submitted by 3:00 pm, April 1.
Applications that are received but do not include ALL paper and online information by 3:00 pm, April 1 will be
considered incomplete and will not be reviewed. |

RETURNING SES providers Login: for potential Supplemental Educational
Service Providers who have previously registered but not submitted their applications.
Select your Provider Name from the drop-down list. This is the name that you registered as your SES Provider Name
the first time into the application. Then enter your Password where indicated. This password is the
password you entered the first time into the application and is 5-8 characters in length and is case sensitive.

All NEW potential SES providers: must register to obtain
access to the online application which must be completed by all potential Supplemental Educational Service Providers.
You must "register" so that the system can connect you to the correct application, should you need to return.
The first time into the application, enter your Provider Name where indicated.
This is the name that will be registered as your SES Provider Name. Then, enter a password, of your
own choosing, where indicated. This password should be 5-8 characters in length and will be case sensitive.
