Supplemental Educational Services (SES)
Provider Application

The SES provider application process in Wisconsin involves submitting information both electronically and in print. Applications must include both paper documents and online information submitted by 3:00 pm, April 1. Applications that are received but do not include BOTH paper and online information by 3:00 pm, April 1 will be considered incomplete and will not be accepted.

Submitting Information Online:
The SES Online Provider Application requires the applicant to submit Provider Contact Information, Service Information, Program Information, Program Abstract, and Evidence of Effectiveness. Applications that do not have all of these sections completed can not be submitted. All sections must be completed and saved before submission. All sections can be revised as many times as needed, until the entire completed application is submitted. After an application has been submitted it can NOT be revised.

To save your work for each section but not submit the application, click at the bottom of each page before continuing to the next page or exiting. You do not need to complete the entire application in one sitting. You may log back in at a later date using the Provider Login and password you created on the Provider Login page.

Submitting Paper Documents:
The application is considered incomplete until electronic surveys for Assurances and Evidence of Financial Soundness, Certification of Debarment, and documentation of financial soundness are received by the Department of Public Instruction.

Documentation of financial soundness is a GAAP audit or a letter of engagement to provide a GAAP audit, depending on the entity’s status. More information may be found at http://dpi.wi.gov/esea/pdf/ses_fin_sound_guidance.pdf.

Application Review and Notification:
Online application submissions, electronic surveys, and documentation of financial soundness will be reviewed after the April 1 deadline. Applicants will be notified of approval/denial status approximately 4-8 weeks after the deadline.

Paper documents must be sent via US mail. Faxes will not be accepted.
