Data Collection Plan: HEADER IMAGE


The list of data collections should contain the following information:
  • The Form Number, Title, and any applicable Comments.

  • The Due Date will display whether their is a Specific Due Date, Various Due Dates, or No Due Date Assigned. To see the specific due date(s) for a particular form, select the form number from the drop-down list on the main menu.

  • The Respondent will describe the primary users or respondents of the selected form.

  • Only recurring External and Local Usage forms are listed in the Data Collection Plan.
    Local:  Distributed by DPI for local use only and not returned to the DPI.
    External:  Sent outside DPI to be completed and returned to the department.

  • The Extent of the form describes whether or not all districts are responsible for completing the form.
    Forms requested for completion by all school districts carry an All Districts identifier in this field.

  • The DPI Division and Team responsible for the particular form.

  • The Primary and Secondary Contact and Phone number of the DPI person responsible for collection of the data.

  • The Update Status describes whether the form is new or revised, along with the most current Revision Date.

  • The Response Method explains one or more ways the form can be accessed.
    Print Only Form is a document that can be viewed through your browser but must be printed and then filled out manually. The document can be a MicroSoft Word file MicroSoft Word File, a MicroSoft Excel file MicroSoft Excel File,  an Acrobat PDF file Acrobat File, or an HTML file HTML File.
    Electronic Fillable Form is an electronic fill-enabled version of the form. For example, a fill-enabled Microsoft Excel or fill-enabled MicroSoft Word document that can be completed and submitted on-line. The document can be a MicroSoft Word file MicroSoft Word File, a MicroSoft Excel file MicroSoft Excel File, an Acrobat PDF file Acrobat File, or an HTML file HTML File.
    Web Application Web App File is a web-based data collection application that must be completed and submitted on-line. DPI web applications (e.g., SPR Report, Staff Report, etc.) are also listed at http://www2.dpi.state.wi.us.
