The Data Collection Plan has been developed to provide a convenient reference of forms to be collected from school districts during the year by the Department of Public Instruction (DPI). The Plan lists recurring forms that have been authorized for collection to meet the needs of programs administered by the department. One-time DPI surveys or questionnaires that arise due to special program needs or legislative mandate generally cannot be anticipated. Such nonrecurring DPI forms are not included in the scope of this Plan.

View Data Collections Information

Click here List of All Forms to view the entire list of Data Collections for the Department of Public Instruction.
 

Search by keyword in the form title or comments to view a list of data collections with title or comments that include the search criteria, then click the Search button.
Search by keyword:

To view the list of all data collections for a particular user type or respondent, select a primary respondent from the drop-down list below and then click the Select button.
Select primary respondent:

To view the list of all data collections due for a particular month, select a month from the drop-down list below and then click the Select button.

To view the list of all data collections for a particular DPI team, select a team/program from the drop-down list below and then click the Select button.

To view a particular data collection form when you know the form number. Select the form number in the drop-down list below and then click the Select button.

To view various surveys, questionnaires, sample forms, and miscellaneous forms, select from the drop-down list below and then click the Select button.
